OTTAWA -- Jack Moon, a father of three with a fourth child on the way, has been identified as the victim of a multi-vehicle crash on Highway 401 near Brockville Wednesday afternoon.

Moon and his family were in the middle of rebuilding their lives after a fire destroyed their home in December, according to a GoFundMe set up by a relative.

“This tragedy for my sister has unfortunately gotten worse,” Aisha Summers wrote on the fundraising page this morning.

“At this time any donations are going to be even more appreciated as yesterday unexpectedly they have lost Jack in a car accident and my sister is now having to figure things out without him in their lives.”

Summers asked the community to keep the family in their thoughts and prayers.

Moon was driving back to the Kington area from Ottawa, according to his Facebook page. He was in the capital region trying to get passports for his family for a trip to Disney. He posted that the passports had been damaged in the earlier blaze.

OPP continue to investigate Wednesday’s multiple-vehicle pileup that happened as a snow squall moved through the region.

Moon leaves behind his fiancé, Krystiannah Summers, three children and one child on the way.
